Requirements Checklist
These are general requirements, and every application is different. Some documents may not be required, while additional documents may be recommended based on your individual circumstances. During your appointment, we'll review your application and guide you on the documents needed to help strengthen your application.
- Valid Jamaican passport (valid for at least 6 months beyond intended stay)
- Recent passport-sized photograph (2" x 2", white background)
- Letter from a medical professional in Jamaica describing the condition and need for treatment abroad
- Letter from the United States medical provider confirming acceptance, proposed treatment, and estimated costs
- Proof of funds or sponsorship to cover treatment, travel, and living expenses
- Evidence of accommodation and travel plans
- Proof of employment, business, family, property, or other ties to Jamaica
- Medical records relevant to the proposed treatment, where appropriate
Important Note
A medical-purpose B-2 application requires strong evidence of the treatment plan, payment arrangements, and intent to return to Jamaica after treatment. An interview may be required.
